2. Create a new wiki page for your plaque on the new 3DSP site.
Note: Please title your new page in the following way: Znameofsitetitleofyourplaque
Example: "Zchapelreverendorrootdoctor"
Enter the URL of your wiki page you created for the new 3DSP site.
Note: choose "small" in the area that says size.
Click generate image
right click on the image of the QR code, rename and save the image in the following way: QRimage-nameofyourplaque.jpg
save your QR image on your new wiki page
5. Post an image of your plaque and QR code on the appendix page. Use the following instructions to post an image of your plaque
Go to the following page: http://3dsp.wikispaces.com/Z--appendix-of-QR-codes
Follow the directions posted on the page.

2. Create a new wiki page for your plaque on the new 3DSP site.

3. On your new page post the following items:
- image of your plaque. CLICK HERE for the template that creates a jpeg image of your plaque.
- well edited text of the extended story behind your plaque.
- a jpg image of the QR code for your plaque page (see instructions in the next step).

5. Post an image of your plaque and QR code on the appendix page. Use the following instructions to post an image of your plaque
